On April 12th, the Practice Transformation Workgroup kicked off with a robust discussion on data, metrics, roles and expectations. Carol presented a compelling presentation on how the PCHM Model is the initiative that not only envelops Transformation of Care, the model additionally aligns with all the Medicaid Demonstration Project areas. The providers and other members were engaged and contributed to discussions related to measures during Wes’s data presentation. Wes presented great data that thoroughly pointed out the disparities, population health and some overall statistics within the region.
During the April 26th Practice Transformation Workgroup Meeting, the Readiness Assessment was discussed. The group wanted to ensure the assessment is clear and concise to avoid confusion when completing the document. Next steps for the PTW include completing the Readiness Assessment based on the comments received, finalizing the CSA, gathering feedback from the PTW on the weighted criteria, revising and sending out the letter to be included with the assessments and lastly sending out the notice of payment to provider organizations. GCACH is thankful for the participation of the group members and are excited for the work ahead!
