Title: Practice Transformation Navigator

Reports to: Director of Practice Transformation

FLSA Status: Exempt

FTE Status: 1.0 FTE

Hours: Monday through Friday

Location: Kennewick, WA

Salary: DOQ ($60,000-$65,000)

Travel: Up to 75%

Job Summary

  • The Practice Transformation Navigator is part of a team that implements interventions at the practice level (e.g. clinical sites, hospitals) to improve clinical quality and efficiency and supports the practice team during stages of Practice Transformation. They are responsible for working with practices to facilitate redesign efforts, support care teams, introduce population health management and support achievement towards improved processes and clinical outcomes. The goal of coaching is to progress toward a desired practice level of transformational change.

Major Duties

Responsibilities and Duties

  • Act as a liaison, teacher, facilitator and technical assistance provider for a subset of enrolled clinicians and their associated practices/agencies/facilities (practices) participating in Practice Transformation
  • Provide on-site and remote technical assistance and practice facilitation to the clinicians, care teams and their associated practices per a set of intervention work plans to help practices improve quality measures and transformation in patient and family centered care design.
  • Provide continuous, data driven quality improvement sustainable business operations.
  • Facilitate trainings and practice guidance
  • Technical assistance includes workflow design, EHR optimization, assessments, analysis, reporting and other work plan items.
  • Participate in coach workgroups and other learning sessions to share learnings and gain knowledge to better provide facilitation/teaching and technical assistance
  • Participate in providing quality measures and completing practice assessments as required
  • Maintain profiles and work plans
  • Actively participate as part of a team with GCACH to conduct work and resolve any issues
  • Establish and maintain a supportive role with Practice Transformation staff, network partner management, coaching teams, assigned clinicians, care teams and their associated practices

Qualifications and Skills

  • Project management experience
  • Operations experience in a physician’s office
  • Clinical workflow development and /or redesign
  • Experience with PCMH standards and recognition programs
  • Experience with team-based care and population health management and tools
  • Experience with complex care program design and implementation
  • Experience with EMR preferred
  • Working knowledge of HEDIS and CAHPs
  • Experience with quality improvement
  • Experience with process improvement - LEAN experience desirable
  • Knowledge and understanding of population management
  • Strong project management, communication and relationship management skills that can be used in a fluid program environment
  • Able to implement PDSA or like quality improvement methodologies using a data driven method
  • Have a successful history of implementing clinical workflow development or redesign in practices that have electronic health records
  • Ability to lead a care team to function successfully
  • Highly organized and able to handle multiple priorities
  • Excellent oral and written communication skills
  • Excellent interpersonal skills, diplomacy, coaching and change management skills
  • Good Microsoft application skills (Word, Excel and PowerPoint)

Stakeholder Engagement

  • Identify stakeholders, effective methods of stakeholder engagement and processes for optimal collaboration related to program(s) and service(s).
  • Communicate and coordinate with key stakeholders and customers, including staff, management, advocacy groups, consumer groups, etc. to meet GCACH program goals and objectives.
  • Manage stakeholder engagement activities, including drafting regular updates, reports on events, activities and accomplishments for internal and external audiences.
  • Identify training, tools, and material deficiencies within GCACH programs and services and develop and/or recommend improvements.

Administrative Duties

  • Act as member of GCACH Team, including varied contributions to general management.
  • Other duties as assigned.



  • Bachelor’s degree from an accredited college or university in health sciences, public administration, environmental health, health education, or a related field preferred. Substitution: A combination of education and related experience in population health management, community assessment, and data aggregation may be substituted for the bachelor’s degree on a year for year basis.
  • Must own an operable personal vehicle


GCACH covers 100% of all medical and dental benefits for the employee only.

GCACH’s Time-Off Benefits are as follows:

  • PTO – 14 days total per year
  • Sick leave – 6.5 days total per year (if you work 80 hours for 26 pay periods – it is accrued by 1 hour per every 40 hours)
  • Holidays – 10 days
  • Total – 30.5 possible hours of time off
