In May 2018, Greater Columbia ACH sent out the Current State Assessment (CSA) to be completed by organizations wishing to participate in Practice Transformation. Of the 57 CSAs received, GCACH contracted with 23 organizations to serve as the initial cohort of partnering providers. Amongst the organizations selected are several exemplar organizations that serve as models for other providers.
During the March Leadership Council Meeting/March Learning Collaborative, attendees will be hearing from exemplar and other knowledgeable organizations discussing topical areas specifically related to practice transformation. Exemplar organizations include Federally Qualified Health Centers (FQHCs) residing in the Greater Columbia Region that have successfully achieved bi-directional integration of care. As part of the practice transformation process, the exemplar organizations are asked to provide consultation and technical assistance to other organizations going through practice transformation.
Please note, the March Learning Collaborative will coincide with the March Leadership Council Meeting. The convening will take place at Columbia Basin College, room L102, from 9:00 am – 11:30 am on Thursday, March 21, 2019. Suggested attendees include Quality Improvement (QI) staff, Administrators, Managers, Panel Managers, Behavioral Health (BH) Directors and others wishing to learn more about the practice transformation process.
